Confirmation of Registration:
You will receive confirmation of registration via email once receiving both registration form and the proof of payment. If you do not receive a confirmation by October 15, 2011, please contact registration department at E-mail: registration@aao2011.com or please bring a copy of your registration form and proof of payment to the registration desk.

- Payment by credit card is available ONLY through the on-line registration.
- All service charges on credit card are to be responsible by registrants.
- The actual debit amount is subject to change according to the exchange rate.
Terms & Conditions:
- This registration will be completed upon received of full payment.
- Registered delegates who are unable to attend will received 50% refund. A written notification is required to submit to AAO2011Registration Office at registration@aao2011.com before October 30, 2011.
- All approved refunds will be processed and issued 60 days after the congress.
- Transfer to another name is accepted with written notification before October 30, 2011.
